At your first appointment you will need to fill out a medical history form. This means you should have information about your medical conditions and medications that you are currently taking. You will need to provide a list of current medication and medical and dental history such as illnesses and dental care and surgery you have had.
You also need to provide:
- any referral letters from GP or specialists
- Health Insurance Card
- Medicare Card
If you are a DVA patient, you will need to bring in your DVA Gold Card.
If you are a SADS patient, you will need to bring in the form issued by SADS - such as GDS or PDS forms.
