Pensioner Denture Scheme (PDS) - Repair Form

A PDS repair form is designed to repair your current denture.

Approach your local SADS clinic and enquire about your eligibility.

If you are eligible, you may be issued with a PDS form from SADS.  You can take this form to the dentist of your choice who participates in the scheme.

If you gave a PDS Repair Form

We can assess what repairs are required for your dentures and the fee that would need to be paid according to the SADS schedule of fees.  Please note that sometimes a repair cannot be undertaken for a variety of reasons.  The dentist will give you information whether he will be able to help you.  We will also inform you of your contribution before commencing the repair.

At your appointment

If you are issued a PDS repair form, check how long this form is valid.  You need to make sure you can attend your dentist of choice well before the cut off date.  Please call us if you require further information.

You must present your PDS form at your appointment.  You will also need:

- a current pensioner concession card (PCC) or

- a current Health Care Card (HCC)

Please check the date on your card to ensure it is current.
